In ARDS management during mechanical ventilation, the aim is to deliver enough oxygen while reducing ventilator-induced lung injury. The best target set uses a conservative, safe oxygenation range, maintains adequate perfusion, and lets PEEP be adjusted based on FiO2 to meet oxygenation goals.

Specifically, targeting SpO2 around 92–96% with PaO2 about 60–80 mmHg keeps tissues well-oxygenated without pushing for excessive oxygen levels, which can be harmful. Maintaining a mean arterial pressure of at least 65 mmHg helps ensure organ perfusion, which is crucial when adjusting ventilator settings. PEEP should be titrated alongside FiO2 to achieve the desired oxygenation rather than using a fixed PEEP, because this allows gradual alveolar recruitment while avoiding overdistension and hemodynamic compromise.

This approach aligns with lung-protective strategies and real-time oxygenation needs. Other options either tolerate too low oxygenation, rely on a fixed PEEP that may under- or over-distend the lungs, or accept hypotension, all of which are less favorable for guiding ARDS management.
